Family Nurse Practitioner:
JFCS is currently in search of a Family Nurse Practitioner to provide primary-care services for patients of all ages at our Glendale Healthcare Clinic.
** East Valley **
Perform out of office care coordination; facilitate CFT meetings; and provide home- or community-based therapeutic and supportive services in accordance with agency procedures. Perform administrative tasks in accordance with agency procedures.

The Child Clinician will provide clinical oversight, direct therapy and case management services including intake and assessment to clients and families. As well as CFT facilitation, and discharge planning and coordination with other agencies. The clinician will perform administrative tasks in accordance with agency procedures.

**Glendale**
The Child Case Manager provides ongoing case management services to clients and families. This person performs out-of-office case management services and administrative tasks in accordance with agency procedures and the Arizona Principles for working with families and adult clinical teams. The Case Manager will also perform duties in the community and is expected to provide transportation, when necessary, to clients and coordinate transportation services.
