The Child Case Manager provides ongoing case management services to clients and families. This person performs out-of-office case management services and administrative tasks in accordance with agency procedures and the Arizona Principles for working with families and adult clinical teams. The Case Manager will also perform duties in the community and is expected to provide transportation, when necessary, to clients and coordinate transportation services.

Provide clinical oversight, direct therapy and case management services including intake and assessment to clients and families. CFT and MFT facilitation, discharge planning and coordination with other agencies. Perform administrative tasks in accordance with agency procedures.

Nurturing Parenting Practitioner: Family Consultants in the Nurturing Parenting Program provide home-based and community-based services in Maricopa and Pinal Counties to families referred by the Department of Child Safety (DCS). Our consultants work with these families to promote positive parenting ideas and techniques.

Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing service needs, including both primary care and behavioral health. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.
