Overview

** EAST VALLEY CLINIC ** 

 

Treatment Coordinator: this role provides ongoing case-coordination services with a solution/strengths-based orientation to assist in addressing the client and family’s ongoing service needs, including both primary care and behavioral health.

Responsibilities

  • Responds to initial crisis calls and identifies needs & necessary resources accordingly.
  • Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services and demonstrates a positive, strengths-based attitude of teamwork and shared leadership.
  • Performs ongoing case management and coordination of services to children and adults within the Child & Family Team process in accordance with the Plan.
  • Assists clients and families in obtaining community support, identifying resources, and resolving barriers.
  • Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety, Probation/Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, schools, and other providers.

Qualifications

  • Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered
  • Relevant experience working in a behavioral-health setting
  • Level 1 Fingerprint Clearance Card
  • Verification of education, employment, references
  • MVR matching JFCS driving guidelines
  • AZ Driver's License and Valid Auto Insurance
