Youth & Family Specialist: The Youth & Family Specialist (YFS) provides comprehensive support to children and their families through in-home and community-based interventions, while also providing education to address the client’s therapeutic goals.

 

The services provided in this role may include parent education, living skills training, behavior management training, behavioral observations in various settings, consultations with school personnel, and social skills training.

Responsibilities

  • Provide ongoing case management/care coordination services to clients and families. Performs in-office and out-of-office care coordination and provides home or community-based therapeutic and supportive services in accordance with agency procedures.   Performs administrative tasks and case management documentation in accordance with agency procedures.
  • Serves as facilitator of Child & Family Team (CFT) meetings and completes all CFT documentation in a timely manner. Communicates the CFT process to families and conducts the necessary tasks to help families thoroughly understand the CFT process. Makes decisions about the selection of CFT participants.  Performs necessary tasks to support the development of high-functioning CFTs, including outreach and engagement efforts.
  • May be required to facilitate psych-educational groups.
  • Performs ongoing case management/care coordination services to children with serious behavioral health issues and their families in accordance with the Child and Family Team process and the Behavioral Health Service Plan. Assists clients and families in obtaining community support, identifying resources and resolving barriers.  Supports the most appropriate and least restrictive levels of care.  
  • Completes all appropriate documentation, including contact notes, BHSP, SN&CD, Crisis Plans, completing of DSP referrals, SCA, ASR, etc.
  • Works effectively within the System of Care Model to address the client and family’s needs as outlined by the 12 principles including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness.
  • Works collaboratively with the CFT, medical, treatment, and management teams in implementing agency policies and procedures.
  • Acts in a manner that is in accordance with current ethical standards, realizing professional boundaries and protocol that promote a positive public image for the organization and the profession. All actions will be in accordance with applicable state and federal laws and the established professional standards.
  • Performs other related job tasks as assigned.
  • May conduct assessments, screenings and intakes for the clinic if properly credentialed and privileged.

Qualifications

  • Bachelor’s Degree in a behavioral health-related field; OR a Bachelor’s Degree in any discipline with at least one year full-time direct behavioral health work experience; and or Associates Degree with two years of experience, or high school diploma with 4 years of case management experience.
  • Behavioral health experience; Inpatient/Residential experience; or Group Home/DDD/CPS in-home work experience.
  • Ability to adjust to changing and shifting priorities and duties for the benefit of the program.
  • Ability to work a 40-hour flexible schedule, including some nights and weekends.
  • Must possess strong verbal and written communication skills.
  • AZ DPS Level 1 Fingerprint Clearance Card.
  • AZ Driver’s License and valid Auto Insurance meeting organization requirements.
  • Bilingual, Bicultural a plus.
