Overview

 

 

 

Provide ongoing case management services to clients and families that are receiving psychiatric services.  Perform case management services; facilitate Child and Family Team process for children and administrative tasks in accordance with agency procedures.

Responsibilities

  • Provides ongoing case management/case coordination services that foster solution/strengths-based orientation to assist in addressing the client and family’s ongoing service needs.   Responds to initial crisis calls and identifies needs and necessary resources accordingly. Uses established best practices to engage formal and informal resources for the client including the transition, discharge and aftercare plans of behavioral health services and demonstrates a positive, strengths-based attitude of teamwork and shared leadership.
  • Performs the duties of case manager by providing ongoing case management and coordination of services to children and adults within the Child and Family Team process in accordance with the Behavioral Health Service Plan. Assists clients and families in obtaining community support, identifying resources and resolving barriers to address their needs. Completes reports including BHSP, Crisis Plans, SCA, ASR and annual updates.
  • Provides ongoing case management for the coordination of behavioral health services with other agencies, such as Department of Child Safety, Probation and Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, schools and other providers.  Participates in assigned JFCS and job related committees (BH Matrix; Training; Substance Abuse; Utilization Management; Safety, MIS/NextGen Computer, and Risk Management).
  • Works effectively within the Arizona Recovery Model to address the client and family’s needs as outlined by the 12 principles including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness.

  • Works collaboratively with the CFT, medical, treatment and management teams in implementing agency policies and procedures, acquaints the agency with community trends, identifies gaps in services and works with special projects and committees to better meet the client’s needs. Identifies, prioritizes and records the family vision and goals via the Behavioral Health Service Plan, and all other related clinical documentation.

Qualifications

  • Bachelor’s Degree in behavioral health related field  and relevant direct behavioral health work experience. 
  • Must be eligible to be Credentialed and Privileged by Maricopa County RBHA, preferred. Must be eligible to obtain and maintain Level One Fingerprint Clearance Card. Behavioral health experience required; knowledge may be acquired through work experience and/or internship.
  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • AZ driver's license
  • Valid AZ auto insurance
  • Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad.
