**East Valley (Mesa/Gilbert) Location.**   Clinical Coordinator - Adult SMI Services: The Clinical Coordinator will train new case-management staff and serve as point person for procedural questions pertaining to the Adult SMI Program working out of our East Valley Healthcare Center.

**East Valley Clinic   Adult SMI Case Manager: The Adult Case Manager (ACM) provides ongoing case management services to members designated as Seriously Mentally Ill (SMI) who are receiving ongoing behavioral healthcare.

- West Valley Clinic Medical Assistant (CMA): Our West Valley Healthcare Center is seeking a capable and experienced Certified Medical Assistant to work with the site medical staff in addressing matters related to the provision of primary care & psychiatric services to JFCS clients.

- East Valley Clinic Adult SMI Case Manager: The Adult Case Manager (ACM) provides ongoing case management services to members designated as Seriously Mentally Ill (SMI) who are receiving ongoing behavioral healthcare.

** East Valley Location **   Integrated Health Navigator - Adult SMI Services: The SMI Integrated Health Navigator provides coordination for one‐on‐one primary care services and behavioral-health services for Adult SMI members, while also establishing relationship bonds that encourage clients to live and work in a fulfilling fashion.

- Central Phoenix Clinic Adult SMI Case Manager: The Adult Case Manager (ACM) provides ongoing case management services to members designated as Seriously Mentally Ill (SMI) who are receiving ongoing behavioral healthcare.

**West Valley Location**   Adult SMI Case Manager: The Adult Case Manager (ACM) provides ongoing case management services to members designated as Seriously Mentally Ill (SMI) who are receiving ongoing behavioral healthcare.
