Overview

Youth in Transition Program:

 

Transition Facilitator: The Transition Facilitator in the Youth in Transition Program will assist youth who are transitioning into the adult behavioral-health system by providing transitional learning opportunities, coaching, and overall support. This position helps youth in identifying and overcoming obstacles to living independently, obtaining employment, and obtaining post-secondary education.

Responsibilities

  • Provides adequate transition support while youth continue into the GMH/SA system, teaching independent living skills via community resources, and developing supportive social and recreational activities.
  • May provide clients with transportation in the community.
  • Conducts strength-based needs assessments related to transitioning youth.
  • Coaches youths as they progress through levels of service.
  • Identifies obstacles to living independently, obtaining employment, obtaining post-secondary education, and in developing supportive social and recreational activities.
  • Develops strategies to assist youth in overcoming barriers.
  • Assists youth in acquiring resources such as housing, employment, social security benefits, and Primary Care Physician services in support of a whole health integrated care approach.
  • Collaborates with the SMI case manager, educators, and other service practitioners as needed to ensure appropriate services and support for clients.
  • Develops support plans and promote involvement of parents in planning services.
  • Develops and maintains community relationships to assist improving access to community services.
  • Refers youth to appropriate clinical, behavioral specialists as needed.
  • Charts and records services rendered. Collects data relevant to progress of youth across their goals, and at each active status along treatment path.

Qualifications

  • Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered.
  • Bilingual preferred (Spanish-English). Pay Differential offered.  
  • Relevant experience working in a behavioral-health setting.
  • Capability to obtain a Level 1 Fingerprint Clearance Card
  • MVR matching JFCS driving guidelines.
  • AZ Driver's License and Valid Auto Insurance
