The Integrated Health Navigator is responsible to coordinate care with physical health providers and serve as client advocates.  Navigators will help to remove personal and environmental obstacles to health care access.  Navigators provide a set of non-clinical activities that engage, educate and offer support to individuals, their family members, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up. 

Responsibilities

  • Provide intensive one on one services for each client, with at least bimonthly in-person contacts throughout the year and more for those needing additional support. 
  • Coordinate physician visits and other medical appointments. 
  • Encouraging a stable and regular relationship with a primary care provider. 
  • Assist clients in obtaining  preventative screening (including cholesterol, diabetes, heart disease, asthma and breast cancer screening)
  • Offer self-management workshops and educate clients around nutrition, healthy eating and physical activity. 
  • Provide education about medical conditions and recovery strategies. 
  • Arrange transportation to and from medical services.
  • Accessing and maintaining insurance coverage.
  • Facilitate communication with health care providers.
  • Participate in site-based staff meetings and interdisciplinary project teams
  • Responsibility to comply with all pertinent regulations under HIPAA, OBHL and the contract.

Qualifications

  • Bachelors Degree Preferred
  • Must be able to communicate verbally and in writing. 
  • 1-2 years of experience of community behavioral health system required. 
  • Knowledge of primary care practices preferred. 
  • Knowledge of community health resources and how to help clients take advantage of programs that will improve their overall health status.
  • Must be able to plan, organize and prioritize multiple activities.  Flexibility is a critical skill. 
  • Computer proficiency required.
  • Bilingual in Spanish preferred.
  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • Reliable Transportation
  • Valid AZ Auto Insurance
