Professional Clinician - CCHT: this role provides intake and comprehensive assessment services, home-based therapy, and case management services to children & families/caregivers who are experiencing crisis. Service area is within Maricopa County and parts of Pinal County (Apache Junction and San Tan). Services are provided to children and families who need short-term crisis intervention and stabilization services to remain in the community. There is a mix of virtual and in-person service delivery methods.

Responsibilities

  • Provides in-home or community-based services within the JFCS service/demographic area (Maricopa County and parts of Pinal County).
  • Ability to work a flexible schedule that includes evenings and at least one weekend day (shift).
  • Provide crisis intakes, service planning and effective crisis planning with all clients/families.
  • Provide individual and family therapy (generally short terms solution focused, CBT and DBT) to assist in identifying and providing viable interventions to prevent hospitalizations, re-hospitalizations or disruptions.
  • Provides parenting education, psychoeducation, diagnosis education, mental health education and behavioral health system education for parents/caregivers.
  • Provide case management, coordination, attend CFT’s and service linkage.
  • Assists in addressing the client and family’s ongoing needs. Uses clinical best practices to engage formal and informal resources for the client including the transition, discharge and aftercare plans of behavioral health services.
  • Demonstrates a positive, strengths-based attitude of teamwork and shared leadership.
  • Ensures all services are clinically necessary and are documented as such within 24 hours at least 85% of the time (prefer concurrent documentation).
  • Works collaborate with assigned Crisis Youth and Family Specialist to work on skill attainment and service plan achievement.
  • Prepares required reports, inclusive of incident reports, DCS reports, performance audits, and compilation of meeting minutes.
  • Provides coordination of behavioral health services with other agencies, such as Department of Child Safety, Probation and Parole Departments, Division of Developmental Disabilities, other behavioral health providers, schools and others.
  • Provides skills training and education for co-workers/teammates.

Qualifications

  • Clinician V: Independent License conferred by the AZBBHE (LCSW, LPC, LMFT)
  • Clinician IV: Associate License conferred by the AZBBHE (LMSW, LAC, LAMFT)
  • Experience working with Older Adults/Seniors
  • Must be able to obtain a Level 1 Fingerprint card without restrictions.
  • Reliable transportation
