  • Overview

    Jewish Family & Children’s Service has been strengthening our community by providing
    Behavioral Health, Integrated Healthcare and Social Services to all ages, faiths and backgrounds
    since 1935.


    Critical Tasks


    • Identify the client population most in need of engagement through risk stratification and predictive analytics
    • Disseminates actionable data to appropriate staff within the organization
    • Collaborates with clinics to develop and implement processes addressing gaps in care
    • Coordinates and manages various programs, projects, clinical initiatives and projects related to care coordination
    • Utilizes Population Health application
    • Enters data into Health Plan portals




    • Qualifications

      • Bachelor’s degree in health care field / combination education & experience
      • Care Coordination/Disease Management/Care Management experience desired
      • Excellent attention to detail, required analytical and investigative skills
      • Requires critical thinking, communication, influence, decision making, analytical and flexibility skills to make optimal decisions based on multiple variables and desired outcomes
      • Demonstrates ability to build collaborative relationships and to think innovatively to solve problems and promote optimal quality of care and service
      • Demonstrates initiative in achieving individual, team and organizational goals and objectives
      • Demonstrated skills in leadership, communication, quality improvement, team building, problem solving, and change management
      • Must be willing and able to travel to all Practice Locations
      • Able to work independently with limited direction
