    Behavioral Health, Integrated Healthcare and Social Services to all ages, faiths and backgrounds
    since 1935.

    Responsibilities

    Critical Tasks

     

    • Identify the client population most in need of engagement through risk stratification and predictive analytics
    • Disseminates actionable data to appropriate staff within the organization
    • Collaborates with clinics to develop and implement actions addressing gaps in care
    • Coordinates and manages various programs, projects, clinical initiatives and projects related to care coordination

     

    Key Performance Indicators

    • Measuring outcomes of care management initiatives
    • Track performance of quality measures
    • Monitor performance by tracking cost and utilization trends

     

    Metrics

    • Timely completion of tasks/assignments
    • Customer feedback

    Qualifications

    Qualifications

    • Registered Nurse or Medical Social Worker with current active AZ license
    • 3+ years Care Coordination/Disease Management/Case Management
    • Excellent attention to detail, required analytical and investigative skills
    • Requires critical thinking, communication, influence, decision making, analytical and flexibility skills to make optimal decisions based on multiple variables and desired outcomes
    • Demonstrates ability to build collaborative relationships and to think innovatively to solve problems and promote optimal quality of care and service
    • Demonstrates initiative in achieving individual, team and organizational goals and objectives
    • Demonstrated skills in leadership, communication, quality improvement, team building, problem solving, and change management
    • Must be willing and able to travel to all Practice Locations
    • Able to work independently with limited direction
