  • Overview

    The Case Management Aide provides on-going administrative support to the assigned Case Manager / High Needs Case Manager.  The Case Management Aide will act as a liaison for the Case Management Team and Child & Family Team (CFT):

    Responsibilities

      • Facilitates Client engagement in services by conducting reminder calls, doing home visits, collaborating with treatment teams to ensure services are delivered in a timely manner.
      • Observes court hearings and other Clinical Team meetings.  Duties to include taking notes, observation of process and providing Service Plan updates during CFT Meetings.
      • Understands contractual requirements when working in a Medicaid funded system.
      • Learn about youth in foster care and how to coordinate with their legal guardians.  Duties to include understanding DCS / Foster Care Policies & Procedures.
      • Assists in the coordination of Patient Treatment Plans
      • Observes Patient evaluations and assessments
      • Observes the monitoring of patient progress and the adjustment of Treatment Plans
      • Shadows direct service personnel through court cases, school counseling and other interacting agency involvement.
      • Travels with direct service personnel to patient homes
      • Assists with the coordination of wellness checks and monitoring of patient progress
      • Assist in the Agency referral process
      • Assigned Duties subject to change depending on the need of the Agency

    Qualifications

    • Education / Experience Requirements:
      • Currently Enrolled in 2nd Semester of final year earning Bachelors in a Behavioral Health related degree.
    • Ability to pass DPS Level 1 Fingerprint Clearance without any restrictions
    • Reliable Transportation
    • Active / Unrestricted AZ Drivers License without DUI / DWI or reckless driving in the past 39 months
