- Intensely manages a flexible caseload as determined by the Clinical Manager including a mix of approximately 2/3 employment-related rehabilitation and approximately 1/3 psychosocial rehabilitation. The caseload size is determined based on a dynamic review of both the clinical need and resources available.
- Provides and tracks detailed information on all employment-related engagement and caseload.
- Utilizes psychiatric rehabilitation best practices including evidence-based employment practices to assess participant’s needs emphasizing a philosophy for appropriate individuals to reach their desired outcome.
- Educates and assists clients in identifying the resources needed to obtain and maintain employment including groups and workshops.
Provides limited coaching until services for job coaching can be obtained by an
- employment provider agency.
- Utilizes available tools to screen and assess participants for employment and psychosocial rehabilitation opportunities, e.g. Meaningful Community Activity Worksheet (MCAW), readiness assessment, vocational profile, etc.
- Maintains expertise and technical knowledgebase around the following activities:
- Identifying opportunities for participants that are externally and internally supported through employment networks.
- Coordinating care and collaborating with Rehabilitation Services Administration (RSA) staff, vocational and rehabilitation providers, and other stakeholders to ensure preferred and most effective services are provided to clients.
- Under the Vocational and Rehabilitation Services Interagency Governmental Agreement (IGA), works in partnership with assigned RSA Vocational Rehabilitation Counselor(s), community rehabilitation program providers and others involved in assisting client to become involved in or maintain involvement in meaningful community activities or employment.
Maintaining expertise and fulfilling RSA collaborative inter-governmental agencies training requirements.
- Collaborates with the Clinical Manager in identifying participants for the employment and psychosocial caseloads.
- Guides, educates and trains Case Management team members on how to make referrals and coordinate services for routine psychosocial rehabilitation activities. Mentors new Rehabilitation Specialist staff as needed.
Collaborates with the Clinical Manager on oversight and technical support to ensure EA-1013 employment fields are updated and accurate.
- Reports tracking information related to designated employment and psychosocial rehabilitation caseload.
- Provides assistance to the Clinical Manger in developing and maintaining monthly reporting requirements.
- Completes all required tracking and data collection reports as requested/ required by the RBHA.
