  • Overview

    The Rehabilitation Specialist is responsible for providing rehabilitation services to participants, which includes a focus on gaining competitive employment and/or engaging around psychosocial rehabilitation activities or educational goals. This position is also responsible for developing and sustaining a culture of work as an integral part of recovery.

    Responsibilities

     

     

    Clinical Role

    • Intensely manages a flexible caseload as determined by the Clinical Manager including a mix of approximately 2/3 employment-related rehabilitation and approximately 1/3 psychosocial rehabilitation. The caseload size is determined based on a dynamic review of both the clinical need and resources available.
    • Provides and tracks detailed information on all employment-related engagement and caseload.
    • Utilizes psychiatric rehabilitation best practices including evidence-based employment practices to assess participant’s needs emphasizing a philosophy for appropriate individuals to reach their desired outcome.
    • Educates and assists clients in identifying the resources needed to obtain and maintain employment including groups and workshops.

    Provides limited coaching until services for job coaching can be obtained by an

    • employment provider agency.
    • Utilizes available tools to screen and assess participants for employment and psychosocial rehabilitation opportunities, e.g. Meaningful Community Activity Worksheet (MCAW), readiness assessment, vocational profile, etc.
    • Maintains expertise and technical knowledgebase around the following activities:
      • Identifying opportunities for participants that are externally and internally supported through employment networks.
      • Coordinating care and collaborating with Rehabilitation Services Administration (RSA) staff, vocational and rehabilitation providers, and other stakeholders to ensure preferred and most effective services are provided to clients.
      • Under the Vocational and Rehabilitation Services Interagency Governmental Agreement (IGA), works in partnership with assigned RSA Vocational Rehabilitation Counselor(s), community rehabilitation program providers and others involved in assisting client to become involved in or maintain involvement in meaningful community activities or employment.

    Maintaining expertise and fulfilling RSA collaborative inter-governmental agencies training requirements.

     

    Consultative Role

    • Collaborates with the Clinical Manager in identifying participants for the employment and psychosocial caseloads.
    • Guides, educates and trains Case Management team members on how to make referrals and coordinate services for routine psychosocial rehabilitation activities. Mentors new Rehabilitation Specialist staff as needed.

    Collaborates with the Clinical Manager on oversight and technical support to ensure EA-1013 employment fields are updated and accurate.

     

    Reporting/Administrative Role

    • Reports tracking information related to designated employment and psychosocial rehabilitation caseload.
    • Provides assistance to the Clinical Manger in developing and maintaining monthly reporting requirements.
    • Completes all required tracking and data collection reports as requested/ required by the RBHA.

    Develops and participates in partnering relationships in the community, e.g. RBHA, employment provider agencies, and employers.

     

    Outcomes/Targets*

    • Employment:
      • # current caseload applications/participant on average completed weekly.
      • # 50% of current caseload, # employer contacts/participant on average completed weekly.
      • # 30% of current caseload, # interviews completed/participant on average completed weekly.
      • # 80% of caseload competitively employed within 4 months of assignment to employment caseload.

     

    *Baseline employment tracking data will be developed over a 3-month period then benchmarks and target will be adjusted if necessary.

    • Psychosocial:

    # 80% of current caseload engaged in meaningful community activity

    • including academic enrollment within 4 months of assignment to psychosocial rehabilitation caseload.

     

    • Encounter/Productivity:

    # 25 hours equivalent of billable hours weekly on average.

    Qualifications

    Bachelor’s Degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology or a related field.

    • Minimum of 2 years’ work experience in behavioral health, preferably providing vocational and/or rehabilitation services to adults diagnosed with a serious mental illness in an interdisciplinary clinical team environment.
    • Understanding and application of best practices in psychiatric rehabilitation particularly around employment.
    • Must maintain a valid AZ Driver’s License and Level One Fingerprint Clearance Card.
    • Proficiency in Microsoft Office applications.
    • Excellent written and verbal communication skills.
    • Knowledge of psychiatric conditions.
    • Sound judgment and time management skills.

    Ability to follow directions.
