  • Overview

    Provide ongoing care coordination services to clients and families. Perform out of office care coordination in the community up to 50% of the time; facilitate Child and Family Team (CFT) meetings; and provide home or community-based therapeutic and supportive services in accordance with agency procedures.   Perform administrative tasks in accordance with agency procedures


     


     

    Responsibilities

     



    • Provides comprehensive support to clients and their families through in-home and community-based interventions and education to address the client’s therapeutic goals. 
      This may include parent education and skills training, behavior management training, school observations, consultations with school personnel, living skills training and social skills training.

    • Serves as facilitatorof Child and Family Team meetings and completes all CFT documentation in a timely manner. Performs necessary tasks to help families understand the CFT process and make decisions about selection of CFT participants.  Performs necessary tasks to support the development of high-functioning CFTs, including outreach and engagement efforts. 

    • Performs ongoing case coordination services to children with serious behavioral health issues and their families in accordance with the Child and Family Team process and the Behavioral Health Service Plan. Assists clients and families in obtaining community support, identifying resources and resolving barriers. Supports the most appropriate and least restrictive levels of care.   Completes all appropriate documentation, including contact notes, BHSP, SN&CD, Crisis Plans, OOH packets, SCA, ASR,
      etc.

    • Works effectively within the System of Care Model to address the client and family’s needs as outlined by the 12 principles including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness.

    • Works collaboratively with the CFT, medical, treatment and management team in implementing agency policies and procedures.

    Qualifications

    Bachelor’s Degree in a behavioral health related field; or a Bachelor’s Degree in any discipline with at least one year full-time direct behavioral health work experience; Associate’s Degree with at least 2 years full-time direct behavioral health work experience; or a High School Diploma with 4 years full-time direct behavioral health work experience.  Spanish or Arabic Bilingual preferred.  Ability to adjust to changing and shifting priorities and duties for the benefit of the program.  Must be able to work a 40-hour flexible schedule including some nights and weekends.  Must possess strong verbal and written communication skills and be able to engage and work with diverse cultural groups.  Behavioral health experience; Inpatient/Residential experience; or Group Home/DDD/CPS in-home work experience required.
