Provide ongoing case management services to clients and families that are receiving psychiatric services. Perform case management services; facilitate Child and Family Team process for children and administrative tasks in accordance with agency procedures. Bilingual preferred.
Provide ongoing case management/case coordination services that foster solution/strengths based orientation to assist in addressing the client and family's ongoing service needs. Responds to initial crisis calls and identifies needs and necessary resources accordingly. Uses established best practices to engage formal and informal resources for the client including the transition, discharge and aftercare plans of behavioral health services and demonstrates a positive, strengths based attitude of teamwork and shared leadership.
Bachelor's Degree in behavioral health and direct behavioral health experience. Knowledge acquired through work experience and/or internship. Must be eligible to be Credentialed and Privileged by Maricopa County Regional Behavioral Health Authority. Ability to receive and maintain fingerprint clearance card. Bilingual preferred.
