    Responsibilities

    In this role the Nurse Practitioner (NP) will assess health records, immunizations, PCP visits and lab results, medications (Prescription and OTC) and conduct intakes to evaluate children, adolescent and adult clients. The NP will collaborate and coordinate with prescribers for medication, follow up on medical checks and psych evaluations to formulate a current client status. The NP will play an important role as a part of a collaborative team of clinicians, prescribers, medical assistants and medical director.
    Communications with clients, families, Department of Child Protective Services, Providers, Pharmacies, Case Managers, School Districts, Group Homes and Primary Care Physicians will allow the NP to be strategic in assisting with the formulation of treatment plans.

    Qualifications

    The Nurse Practitioner must be licensed as an RN and hold an Advanced Practice Nurse Practitioner certification from the Arizona State Board of Nursing,  Psychiatric/Mental Health Nurse Practitioner certification.   Must hold a current and valid license to practice in the State of Arizona.   Must have full prescription authority and maintain current D.E.A. licensure. Minimum of two (2) years' experience in a Behavioral Health setting or demonstrated equivalent experience.
