Provide ongoing care coordination services to clients and families. Perform out of office care coordination in the community up to 50% of the time; facilitate Child and Family Team (CFT) meetings; and provide home or community-based therapeutic and supportive services in accordance with agency procedures. Perform administrative tasks in accordance with agency procedures.
Bachelor’s Degree in a behavioral health related field; or a Bachelor’s Degree in any discipline with at least one year full-time direct behavioral health work experience; Associate’s Degree with at least 2 years full-time direct behavioral health work experience; or a High School Diploma with 4 years full-time direct behavioral health work experience. Spanish or Arabic Bilingual preferred. Ability to adjust to changing and shifting priorities and duties for the benefit of the program. Must be able to work a 40-hour flexible schedule including some nights and weekends. Must possess strong verbal and written communication skills and be able to engage and work with diverse cultural groups. Behavioral health experience; Inpatient/Residential experience; or Group Home/DDD/CPS in-home work experience required.
