Mesa Location - Perform out of office care coordination; facilitate CFT meetings; and provide home- or community-based therapeutic and supportive services in accordance with agency procedures. Perform administrative tasks in accordance with agency procedures.




  •  Provide in-home and community based intervention and education to support consumers and thier families.
  • Serve as facilitator of Child and Family Team (CFT) meetings while assisting families in understanding the CFT process.
  • Completes all CFT tasks to support the development of successful meetings including outreach and engagement efforts.
  • Completes documentation in a timely manner.
  • Performs on-going case coordination services according to CFT meetings and Behavioral Health Service Plan.
  • Ensures coordination of Behavioral Health Service with third party agencies such as Child Protective Services, Probation and Parole Departments, Division od Developmental Disabilities, and other providers.
  • Additional duties as assigned.


Must have a Bachelor's Degree in a behavioral health related field; or a Bachelor's in any discipline with at least one year full-time direct behavioral health work experience; or an Associates Degree in behavioral health related field with at least 4 years of full-time direct behavioral health work experience.
