Overview

The Quality Coach will support and collaborate with teams at the practice sites to achieve goals and objectives of the organizations Quality Management Program.

Responsibilities

  • Collaborates with practices to develop and implement Population Health Management strategies and programs unique to clinic population.
  • Monitors and communicates outcomes measures to individual practices. Collaborates with the management team to develop corrective action plans when necessary.
  • Monitors and collaborates with site Management Teams on performance improvement initiatives, assists in identifying barriers and creating plans for improvement.
  • Collaborates with Business Intelligence and Quality to generate reports detaining findings, identifying trends and communicating findings to Management team at individual practice sites.
  • Actively participates as a member of the Quality Management Department.
  • Performs other related job tasks or responsibilities as assigned.

Qualifications

  • 5 years health care experience.
  • Bachelor’s Degree in a relevant field preferred.
  • Working knowledge of HEDIS measures and regulatory reporting.
  • Must have a basic understanding of quality improvement processes.
  • Knowledge of Population Health Management strategies.
  • Knowledge of process management.
  • Proven ability to work with people at all levels within the organization.
  • Ability to work in a team environment.
  • Excellent interpersonal, verbal, written, administrative and customer service skills.
