Overview

This position works with the site medical staff in support of and with issues related to provision of psychiatric services to JFCS clients.  The Medical Coordinator works under the direction of the Medical Staff to respond to client concerns about their medications, researches problems that arise, interacts with pharmacies, funding sources and other stakeholders to resolve problems and assists the Nurse in obtaining vitals to include blood pressure, pulse, height, and weight.  The Medical Coordinator performs routine clerical and administrative tasks necessary to resolve these issues and documents all of their activities in accordance with JFCS policies.  The Medical Coordinator may also engage in patient education and support activities.

Responsibilities

 

  • Takes direction and consults with Medical Staff in implementing solutions to medication-related issues, including receiving and handling pharmacy calls, refill requests, medication prior authorizations and additional medication needs. Documents all interaction with clients, Medical Staff, pharmacy, RBHA and other agencies accordingly. May provide patients with information on wellness/health promotions. Completes Suicide Risk Assessment with clients and follows clinic protocol. May complete medication reconciliation prior to prescriber med checks as indicated.
  • Under the direction of the Medical Staff obtains client’s vitals to include height, weight, blood pressure and pulse and records them in the client’s medical record.
  • Provides ongoing medical coordination of services to assist in addressing the client and family’s ongoing medication needs.  Uses established best practices to engage formal and informal resources for the client.  Works within established ethical guidelines.
  • Works collaboratively with the CFT, medical, treatment and management teams in implementing agency policies and procedures, acquaints the agency with community trends, identifies gaps in services and works with special projects and committees to better meet the client’s needs. Identifies, prioritizes and records the family vision and goals via the Behavioral Health Service Plan, Safety Plan, and all other related clinical documentation.

Qualifications

High School diploma or equivalent. Nursing or Medical Assistant Certification required.  Able to obtain and maintain fingerprint clearance in Arizona; Valid driver’s license and proof of auto insurance. General knowledge of behavioral health disorders; General knowledge of psychopharmacology; excellent written and verbal communication skills; detail oriented; able to use the agency’s electronic medical records system. Knowledge of medical terminology. Direct behavioral health and/or healthcare work experience highly preferred.
