Overview

Provides outpatient healthcare services for individuals and families enrolled in Children’s Rehabilitation Services at an integrated healthcare facility. Works as a member of a multi-disciplinary team to meet the clinical needs of the service recipients. Responsible for the provision of clinical assessments and behavioral health interventions in a medical setting. Responsible for treatment planning in conjunction with the child’s/family’s multi-disciplinary team. Demonstrates the ability to perform, analyze and document behavioral health assessments, interventions and services in an integrated setting and/or in the client’s home or community. Maintains working knowledge of community resources and effectively provides information to clients/caregivers. Effectively works with primary care physician to cooperatively meet the needs of clients/families. Will participate in trainings, staffing and supervision as determined to provide necessary education to multi-disciplinary team members, clients/families.

 

 

 

 

 

Responsibilities

 

Diagnosis & Assessment

Performs assessments and triage as appropriate; formulates and documents appropriate diagnoses; coordinates diagnosis with prescribers when applicable. Collaborates with child/family and their multidisciplinary team to communicate results of assessment and diagnosis.

Individualized Service Plan

Develops whole health service plans that are strengths-based, in collaboration with child, family and multi-disciplinary team. Ensures ISPs contain measurable goals and objectives based on the needs identified in the assessment; frequency of service will reflect intensity of assessed need. Ensures discharge goals are clear and attainable.

Therapeutic Interventions

Conducts individual therapy that is relevant to the needs/goals identified in the assessment. Interventions will support clients identified whole health issues. Documentation will reflect a person/family centered strengths based approach. Brief solution focused interventions will be used when clinically appropriate.

 

Conducts family treatment that is goal focused, strengths-based and supports the treatment goals identified in the client assessment and their whole health service plan. Treatment progress will be monitored regularly and documented in the medical record.

 

Provides care coordination to clients and families to assist clients/families in accessing needed services. Coordinates and interfaces with family and/or other caregivers when clinically appropriate.

 

Completes all required paperwork within 24 hours of delivery of service. Enters documentation into the client’s electronic health record in accordance with clinic requirements.

 

Submits documentation within 24 hours of delivery of the service in an accurate manner. Services requiring prior authorization will be processed promptly in order to ensure proper reimbursement of services rendered.

 

Responds to client crisis in a clinically responsible manner. Keeps multi-disciplinary team members apprised of all crisis situations pertaining to client. Observes mandatory reporting requirements as required by statute.

 

 

 

 

Qualifications

 Master’s Degree in a Behavioral Health field.

 

  • Independent License in good standing from Arizona Board of Behavioral Health Examiners required.
  • LCSW preferred. LPC or LMFT required.
  • Experience working with children and families.
  • Excellent verbal and written communication skills.
  • Bi-lingual – Spanish preferred.

Basic computer literacy with a minimum typing speed of 25 wpm.
